We ask that you call when you have a few minutes available so we may gather the information necessary to schedule your appointment. Please have your insurance card or insurance information available when calling.
Telehealth
For your convenience, we provide Telehealth services for patients that are unable to come to the office for their appointment. As the COVID Emergency has ended some insurance may no longer cover telehealth. Be sure to confirm with your carrier directly.
Preparing for your first appointment
To avoid delays during your appointment, we highly recommend you complete the registration from Tebra. You will be receiving a link sent to your email or phone prior to your appointment. Please bring a photo ID, your insurance card and a referral from your primary care physician (if applicable) to your first appointment.
If you did not complete your registration at home prior to your scheduled appointment, please arrive 30 minutes prior to your appointment time to register.
If you have had any X-rays, CT scan, or MRIs performed, please request the report from the imaging facility. They may need your consent to provide these to us. Please bring any pertinent reports, films or CDs with you on the day of your appointment.
If you have had an electrodiagnostic test (EMG/NCV), or any previous spinal injections or surgeries, please provide us with the report.
Upon Arrival
Please check in with our receptionist. Our staff will provide you with a registration tablet and answer any questions you may have about our office policies. Co-pays will be collected prior to being seen, if applicable.
Please remember to put your cell phones on silent or vibrate modes so as not to disturb others around you in the waiting room. While in the examination room, please refrain from cellphone use.
Cancellation / Late Arrival Policy
If you must cancel your appointment, please provide us with 24 hours’ notice. This will ensure we are able to provide other patients in need of an appointment an opportunity to be seen. We reserve the right to charge a $150 cancellation fee for frequent or last-minute cancellations.
If you arrive late for any reason, please check in with our receptionist. We understand unforeseen delays may occur. We will make every effort to accommodate you, however, please understand you may not always be seen right away, as it inconsiderate to other patients who have arrived on time. There may be times when we run late due to unexpected clinical needs. We will attempt to contact you and notify you if that is the case. We respect your time and do our best to stay on schedule.
We want you to be satisfied with the treatment you receive. Please notify your physician or another member of our staff if there is any way we can serve you better.
