It is with mixed emotions that I am informing you that I will be closing my practice, Spine Center and Orthopedic Rehabilitation of Englewood, PC, at 106 Grand Avenue in Englewood, NJ, effective June 30, 2025. I am grateful for the opportunity to have been your physician and for having served this community for the last 30 years. My last day of seeing patients will be June 27, 2025.
Your Medical records will be retained according to New Jersey state regulations. I will be happy to provide your new physician with copies of your medical records. Your medical records are ready for immediate viewing and downloading via the patient portal, which you can easily access from our website by clicking the PATIENT PORTAL ACCESS tab. This is the fastest and easiest way to obtain and/or forward your medical records for your own keeping or forwarding to your new physician. After the practice closes, your records will be maintained by The Physical Medicine and Rehabilitation Center located at 500 Grand Avenue, Englewood, NJ 07631. You may contact them to request records at 201-567-2277 or to schedule an appointment.
I strongly encourage you to find a new provider to ensure continuity of care. I recommend you search the American Academy of Physical Medicine and Rehabilitation website at www.aapmr.org, then got to “Find a Physician” tab to find a board-certified physiatrist in the area. Additionally, your local hospital physician referral service or your insurance company will also list participating providers that are in-network, who may meet your needs.
You may also choose to continue care with my colleagues at The Physical Medicine and Rehabilitation Center as I will be working closely with them and they will be maintaining your medical records.
