Scheduling a New Patient Appointment

Thank you for choosing our practice! To schedule an appointment, please call our office at (201)-503-1900 during regular business hours.

We ask that you call when you have a few minutes available so we may gather the information necessary to schedule your appointment. Please have your insurance card or insurance information available when calling.

How To Prepare For You First Appointment

  • To avoid delays during your appointment, we suggest that you print and fill out the New Patient Forms ahead of time. Please bring the completed forms, a photo ID, your insurance card, and a referral from your primary care physician (if applicable) to your first appointment. You may also fax the above information to our office prior to your visit.
  • If your paperwork is not filled out in advance, please arrive 15 minutes earlier to fill out any necessary patient visit forms.
  • If you have had any x-rays, CT scan, or MRIs done, please have the facilities where testing was administered send us a report. They may need your consent to provide these to us. Please bring any reports, films or CDs with you.
  • If you have had an electrodiagnostic test (EMG/NCV), or any previous spinal injections or surgeries, please provide us with the report.

Upon Arrival

Please sign in with our receptionist. Our staff will obtain necessary information from you, collect co-pay if applicable, and answer any questions you may have about our office policies.

Please remember to put your cell phones on silent or vibrate modes to not disturb others around you in the waiting room. While in the examination room, avoid interrupting the evaluation by answering phone calls.

Cancellation / Late Arrival Policy

If you must cancel you appointment, please provide us with 24 hours notice. This will ensure we are able to provide other patients in need of an appointment and opportunity to be seen. We reserve the right to charge a $50 cancellation fee for frequent or last minute cancellations.

If you arrive late for any reason, please check in with our receptionist. We understand unforeseen delays may occur. We will make every effort to accommodate you, however, please understand you may not always be seen right away, as it not considerate to other patients who have arrived on time. There may be times when we run late due to unexpected clinical needs. We will attempt to contact you and notify you if that is the case. We respect your time and will do what we can to be on schedule.

We want you to be satisfied with the treatment you receive. Please notify your physician or another member of our staff if there is any way we can serve you better.
